# Public Health Alerts in Kochi since 2019: Nipah, Covid and Fevers

> The only Nipah case in Ernakulam district in Wikipedia's record of Kerala outbreaks is that of a 23-year-old student from Ernakulam district, confirmed on 4 June 2019, who recovered. The response put the isolation ward at the Government Medical College, Kalamassery, at the centre of the district's plans, as it would be again in the Covid-19 years. ## Background: Nipah reaches Kerala in 2018

Nipah is a virus carried by fruit bats that can spread to people and between people in close contact. India had outbreaks in West Bengal in 2001 and 2007. Kerala's first came in **May 2018**, when the National Institute of Virology (NIV), Pune, confirmed cases in Kozhikode district; that outbreak, mainly in Kozhikode and Malappuram, claimed 17 lives and was declared over on **10 June 2018**, according to Wikipedia. It is the outbreak dramatised in the 2019 Malayalam film *Virus*.

## June 2019: the Ernakulam case

A year later, the alert came to Kochi. On **4 June 2019** the state confirmed that a **23-year-old college student** from the Paravur area of Ernakulam district, who was studying at Thodupuzha in Idukki, had Nipah. Samples tested at Manipal and at the Kerala Institute of Virology had indicated the infection before NIV Pune confirmed it. He had developed fever after a student internship in Thrissur and was being treated at a private hospital in Kochi. Reports differ on some details of his treatment, and we do not name him.

Contact tracing began at once. Over 300 people who had been in contact with him were placed under observation; reports gave 311, 316 and 327 at different points. Four people who developed fever and sore throat, three who had cared for him at first and a fellow student, were moved to the **isolation ward at the Government Medical College, Kalamassery**, for detailed examination, and the state arranged quarantine facilities there for suspected cases. A team from NIV Pune led by Dr Rima Sahay ran a temporary laboratory at the medical college so that samples could be tested locally.

The suspected cases tested negative. Onmanorama reported on **9 June 2019** that four of 11 suspected patients in Kochi had been discharged, and later reports confirmed further negative results. No second case was found, and the student recovered. Wikipedia lists it as the fourth Nipah outbreak in India.

## Covid-19, 2020–2022

The isolation arrangements tested in 2019 were needed on a much larger scale the next year. ## Later alerts: Nipah elsewhere, smoke and fever seasons

Nipah returned to Kerala several times after 2019, but not to Ernakulam. Wikipedia records a death in Pazhur, Kozhikode district, on **5 September 2021**; six cases and two deaths in Kozhikode in **August–September 2023**; two deaths in Malappuram district in **July and September 2024**; and a renewed alert in **May–June 2025** in Malappuram, Palakkad and Kozhikode, with over 400 people under observation. In total Wikipedia counts 21 Nipah deaths in Kerala between 2018 and 2024.

Other alerts touching Kochi were of a different kind.
